Registering as a patient
The Practice’s list is ’open’ and we are happy to accept new patients living in Runcorn. If you move out of the area, you will not always automatically be requested to change Doctors unless you require home visits as we would be unable to keep you on as patient because of the amount of time the Health Professionals would lose whilst travelling to and from your new address. Registering as a patient is quick and easy with only two brief forms to fill in. These forms are available from Reception.
Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of address such as a recent utility bill.
You will also need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team. These “mini medicals” are usually very informal and involve some routine measurements such as height, weight and blood pressure. Please bring a sample of urine with you when you attend (sample bottles are available from reception). Also, if you are currently taking any medication, please bring along your bottles/packaging in order that the items can be added to your medical records.
Medical treatment is available from the date of registration. Please contact reception for further information.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
